The Temple provides downloadable templates of transliteration booklets for your simcha.  You will need to fill out the information on pages 1, 2, and 11.

You will also need to ask Cantor Neff for a pdf or printout of your child's Torah and Haftarah portions to be added on pages 9 and 10.

Although we happily provide this template for your use, if you should choose to make one, it is at your discretion and expense.
